문제 음주란 건강과 일, 사회적 적응 혹은 다른 기능 영역에서 유의미한 손상을 야기하는 음주 패턴으로서, 알콜 중독의 모든 진단 준거를 충족시키지는 않지만 건강이나 사회적 기능에서 상당한 문제를 낳는 음주를 의미하는 것이다. 본 연구에서는 음주 문제를 지닌 대학생들과 일반인들이 학업이나 사회 생활 뿐 아니라 다른 다양한 영역에서 심각한 문제를 경험하게 될 수 있으므로 문제 음주에 대한 초기 개입이 필요하다고 보고, 인지행동집단치료를 실시하고 그 효과를 검증하였다. 본 연구의 프로그램에서는 음주 문제를 보이는 6명의 대학생과 7명의 일반인 집단을 각각 미시건알콜중독선별검사(MAST)와 알콜사용장애확인검사(AUDIT)를 통해 선발하였고, 7명의 대기자 집단이 통제 집단으로 사용되었다. 인지행동프로그램의 첫 단계에서 치료 참가자들은 음주 기록지(daily drinking cards)를 작성하고, 알콜의 효과에 대한 교육과 치료적 개입을 받았다. 두 번째 단계에서 보다 구체적인 각자의 음주 패턴을 알아보고 음주 목표를 세운 뒤, 알콜과 관련된 신념이 탐색되었으며, 기대에 따른 결과를 알아보고 음주를 대신할 대안적인 행동을 다루었다. 프로그램의 중반 이후부터는 알콜 기대를 변화시키기 위해 집단 내에서 음주를 거절하는 역할 연기가 시도되었다. 연구 결과, 대학생의 경우 인지행동치료집단에서 음주의 빈도 및 양, 통제된 음주 자기 효능감, 그리고 음주 문제가 유의미하게 감소하였으며, 치료를 받은 일반인 집단에서도 음주의 빈도와 양, 알콜에 대한 신념이 유의미하게 감소하였다. 이러한 결과는 알콜에 대한 교육과 알콜 기대 수정, 음주 거절 훈련이 문제 음주의 인지적, 행동적 측면에서 변화를 유도할 수 있으며, 음주 문제에 대한 인지행동치료 프로그램이 효과적이라는 점을 시사한다.영어 초록
Problem drinking is a drinking pattern that produces significant disturbances of health, work, social adjustment, or other areas of functioning and refers to a drinking pattern that does not satisfy all criteria of alcoholism but is of sufficient degree to have resulted in health or social dysfunctioning. The undergraduate students and adults with drinking problem may experience serious problems in various areas of academical, social life, and so forth. Therefore it needs an early intervention. The purpose of this study was to evaluate the effectiveness of cognitive-behavioral therapy program for problem drinking among high-risk undergraduate students and adults. Six undergraduate students and seven adults with drinking problem were selected through MAST and AUDIT, and seven students in the waiting list group were used as control group. In the first stage of the cognitive-behavioral therapy program, all subjects were instructed to record daily drinking cards, and then received education regarding the information of alcohol effects and therapeutic intervention. In the second stage, more specific drinking patterns of each individual member was explored and a drinking goal was established. And then, the related outcome according to alcohol expectancy was explored and alternative behaviors to substitute drinking was sought out. In the latter half of the program, drinking refusal was role-played in the therapy group during 3 sessions in order to change alcohol expectancy. Results demonstrated that in the cognitive-behavioral therapy group, scores of frequency and quantity, of controlled drinking self-efficacy, and of drinking problem in undergraduate students were significantly decreased and scores of frequency and quantity, and of belief for alcohol use in problem drinking adults group were significantly reduced. Overall, these results implied that education of alcohol information, alcohol expectancy modification, and drinking refusal training could induce changes in areas such as cognitive, behavioral aspects of problem drinking and proved the effectiveness of cognitive-behavioral therapy program for problem drinkers.Key words : problem drinking, cognitive-behavioral therapy
