사회불안장애의 이미지 재구성 훈련 : 이론적 배경과 증례 보고

사회불안장애에서 자동적인 정신적 심상이 매우 흔하게 보고되고 있으며, 두려운 상황에서 갑작스럽게 활성화되는 부정적 심상이 장애의 발달과 유지에 매우 중요한 역할을 한다고 알려져 있다. Wild, Hackmann,
Clark(2007)은 사회불안장애 내담자들의 초기 기억과 사회적 상황에서 반복되는 생생한 이미지의 관련성을
확인하여 교정적인 관점을 생성하는 이미지 재구성 작업이 사회불안을 현저하게 감소시킨다고 보고하였다.
본고에서는 이미지 재구성 훈련의 이론적 배경과 기본적인 실시 기법을 구체적으로 소개하고자 한다. 이를
위해 12회기의 집단 인지행동치료에 끝까지 참가했던 이후에도 부정적 평가에 대한 두려움이 쉽게 감소하
지 않았던 29세 여자인 사회불안장애 내담자 1명을 대상으로 집단 치료 직후 2회기에 걸쳐 이미지 재구성
훈련을 실시하였다. 이러한 훈련 후 핵심 신념을 사실로 믿는 정도, 이미지와 기억의 고통 및 생생함 수준
이 다소 감소하였으며, 사회불안의 자기 보고 척도 중 부정적 평가에 대한 두려움 척도(B-FNE) 점수도 다
소 감소하였다. 본 연구에서는 이러한 효과가 나타난 원인과 개입의 의의, 한계점을 논의하였다. 단일사례
연구 설계의 제한점이 많은 바, 이후 보다 많은 사례들의 축적 및 비교연구를 통해 본 치료 프로그램의 효
과가 입증되어져야 할 것이다.
영어 초록
According to current research, automatic and distorted images of the self are commonly repeated in socialanxiety disorder, and sudden activation of these negative images during the fearful situation plays an
important role in developing and maintaining the disorder. Wild, Hackmann, & Clark (2007) suggested
that recurrence of these vivid images in social situation are associated with early memories of
unpleasant/traumatic social experiences. This article presented the theoretical background and the
therapeutic technique of image rescripting training; the exploration of the nature of recurrent imagery in
social anxiety disorder, cognitive restructuring followed by an image rescripting procedure was used to
contextualize and update the memories. A 29-year-old woman who did not respond well to standard social
anxiety disorder Cognitive Behavioral Group Therapy (CBGT) participated in two sessions of imagery
rescripting. The addition of two sessions of image rescripting training in the CBGT led to some
improvement. After two training sessions of image rescripting, the degree of believability on the distorted
core belief, the degree of distressful memories, images and vividness reduced slightly. Also she
demonstrated a slight reduction on Brief-Fear of Negative Evaluation Scale (B-FNE). These positive
changes persisted during three-months follow-up. Limitations of the current study and implications for
future research were discussed.
