본 연구에서는 치료 방법의 발전에도 불구하고 현재까지도 치료가 쉽지 않으며 다수에서 인격장애를 동반하는 강박장애에 있어 스키마치료가 도움을 줄 수 있을 것으로 생각하고 강박
장애 환자의 부적응적 스키마에 대해서 파악하고자 하였다. 연구대상은 일 대학병원에서 강
박장애를 진단 받고 외래 통원 치료 중인 만 18세에서 42세의 환자 38명이며, 정상대조군은
만 22세에서 36세의 대학원생 79명이었다. 스키마를 평가하는데 대표적인 도구인 단축형
Young 도식 질문지(Young Schema Questionnaire-Short Form 3; YSQ-SF3)를 통해 부적응적 스키마
에 대해서 파악하였고, 임상적 증상의 평가를 위해 예일-브라운 강박척도(Yale-Brown Obsessive
Compulsive Scale; Y-BOCS), 백 우울척도(Beck Depression Inventory; BDI), Maudsley 강박행동질문
지(Maudsley Obsessional-Compulsive Inventory; MOCI)를 사용하였다. 나이, 교육정도 및 우울증 점
수를 보정하고 비교한 결과, 강박장애 환자군이 정상대조군에 비해 결함/수치심, 위험/질병에
대한 취약성, 부정성/비관주의의 스키마 점수가 높았다. 또한 일부 스키마 점수는 강박증상과
우울증의 심각도, 그리고 강박증상의 종류와 유의한 상관관계를 보였다. 이러한 결과는 결함/
수치심, 위험/질병에 대한 취약성, 부정성/비관주의의 스키마가 강박장애 환자에서 두드러진
부적응적 스키마일 수 있으며, 이런 영역에 초점을 맞춘 스키마치료의 적용이 필요할 것으로
사료된다.
영어 초록
Despite the development of effective pharmacological and cognitive behavioral therapies, obsessivecompulsivedisorder(OCD) that often combines with deeper underlying pathologies such as personalty
problems and dysfunctional schema, is still difficult to treat. On the assumption that the schema therapy
would provide additional modality to help patients with OCD, we explored maladaptive schemas in
patients with OCD in this study. Thirty eight patients with OCD aged from 18 to 42 years and 79
normal control subjects aged from 22 to 36 years participated in this study. Young Schema
Questionnaire-Short form 3(YSQ-SF3) was used to evaluate maladaptive schemas and the Yale-Brown
Obsessive Compulsive Scale(Y-BOCS), Beck Depression Inventory(BDI), Maudsley Obsessional-Compulsive
Inventory(MOCI) were also used for clinical symptoms. OCD patients showed higher schema scores in
Defectiveness/Shame, Vulnerability to Harm or illness, Negativity/Pessimism than normal controls when
age, education and depression were controlled. Several schema scores correlated with the severity of
depression and obsession as well as symptom dimensions of OCD. These results suggest that
Defectiveness/Shame, Vulnerability to Harm or illness, and Negativity/Pessimism may be pronounced
maladaptive schemas and strategies dealing with these schemas needs to be incorporated in the schema
focused therapy for patients with OCD.
