정신분열병 환자를 위한 게슈탈트 집단치료 프로그램의 개발 및 효과

본 연구는 정신분열병 환자에게 게슈탈트 심리치료 이론을 적용한 프로그램을 개발하고 실시하여, 게슈탈트 집단치료 프로그램이 정신분열병 환자의 공감능력, 대인관계 및 주관적 삶의 만족도를 증진시키고 사회생활기술과 대인관계기능을 향상시키는지를 알아보고자 하였다. 서울 소재 정신보건센터에 등록되어 있는 정신분열병 환자를 대상으로 실험집단과 통제집단에 각 6명씩 배치하여 실험집단에는 게슈탈트 집단치료 프로그램이 90분씩 매주 2회 총 12회기에 걸쳐 실시되었다. 그 결과 본 연구에서 개발한 게슈탈트 집단치료 프로그램 실시 직후 환자 스스로 자신의 상태 및 기능을 보고하는 공감능력, 대인관계능력, 주관적 삶의 만족도 척도에서는 유의미한 차이를 관찰할 수 없었지만 3주 후 실시 된 추후검사에서 공감능력척도에서 유의미한 차이를 보여 이에 대한 지연된 효과가 나타났다고 할 수 있다. 또한 정신보건전문요원이 집단원의 평소 생활을 관찰하여 보고하는 사회생활기술, 대인관계기능척도의 사전 사후 점수는 실험집단이 통제집단에 비해 유의미하게 증가되었음이 관찰되었으며 3주 후 추후검사에서도 그 효과가 지속되고 있는 것으로 나타났다. 그러므로 본 연구는 게슈탈트 집단치료 프로그램이 정신분열병 환자들에게 효과적으로 적용될 수 있는 방법을 제시하였으며 이는 정신분열병 환자의 주관적 보고 결과로는 공감능력 향상에 효과적이며, 정신분열병 환자의 일상생활을 관찰하였을 때의 보고로는 사회생활기술과 대인관계기능 향상에 효과가 있음이 확인되었다.영어 초록
This study showed that an application of gestalt group therapy can improve the empathetic abilities, interpersonal relationship skills, and subjective satisfaction with their own lives of schizophrenic patients, and also promote social skills and interpersonal relationship abilities. Subjects of the study were twelve schizophrenic patients who were registered at a mental health care center in Seoul. They were divided evenly into experimental and control groups. During the experimental period, the experimental group was treated with gestalt group therapy 12 times, two times a week for 90 minutes. After the gestalt group therapy, the patients did not report a significant difference in empathetic abilities, interpersonal relationship skills, and subjective satisfaction with their own lives. However, the mental health care experts did report a significant improvement in the social skills and the interpersonal relationship abilities in the experimental group after observation. Three weeks after the end of the gestalt group therapy, significant improvements were revealed in the empathetic abilities of the patients. The effect of the program was delayed. Also, in interpersonal relationships, tendencies of efficacy were shown, but no efficacy was observed in the subjective satisfaction of patients with their own lives. However, in both the social skills and interpersonal relationship abilities evaluated by experts who observed the patients’ daily lives, some important changes were observed. Thus the effects lasted beyond the experiment. In conclusion, this study presented how gestalt group therapy can be used to help schizophrenic patients. The therapy resulted in effective improvement of empathetic abilities both when patients reported on themselves, and when experts reported on the patients after observation. The therapy was effective in promoting both social skills and interpersonal relationship abilities.Key words : Schizophrenia, Gestalt psychotherapy, Group therapy, empathetic ability, interpersonal relationship skills
