이 연구는 치료적 가창훈련이 뇌졸중 노인 환자의 음성 질과 우울에 어떠한 영향을 미치는지 알아보는데 목적이 있다. 뇌졸중으로 진단 받은 65세 이상의 환자 20명이 연구에 참여
하였으며 실험집단에 10명, 통제집단에 10명씩 연구자의 임의대로 배정되었다. 실험집단 대
상자들에게 호흡 및 발성훈련, 치료적 노래 부르기가 포함된 치료적 가창훈련이 개별 세션
의 형태로 주 2회 30분씩, 5주간 총 10회기로 제공되었다. 대상자의 음성 질과 우울 지수를
평가하기 위해 실험집단과 통제집단에서 사전과 사후에 Praat 프로그램을 이용하여 음성
질의 하위요소인 1)최대발성시간, 2)기본주파수, 3)음성강도, 4)주파수변동률, 5)진폭변동
률, 6)소음대배음비를 측정하였고, 노인우울척도(Geriatric Depression Scale-Korean)검사를
통해 우울 지수를 측정하여 비교분석하였다. 연구 결과, 실험집단에서 최대발성시간, 진폭변
동률, 소음대배음비가 통계적으로 유의미한 변화를 보였다. 기본주파수, 음성강도와 주파수
변동률은 긍정적인 변화를 나타냈으나 통계적으로 유의하지는 않았다. 통제집단에서는 음
성 질의 6가지 하위영역 모두 유의미한 변화를 보이지 않았다. 우울지수는 실험집단에서
평균 0.72점의 감소세를 보인 반면, 통제 집단에서는 우울 지수가 소폭 상승하였다. 이러한
연구 결과는 치료적 가창훈련이 뇌졸중 노인의 음성 질 향상에 효과가 있으며, 우울 감소에
긍정적인 영향을 가져온다는 점을 시사한다.
영어 초록
The purpose of this study was to examine the effect of therapeutic vocal training onelderly stroke patients' vocal quality and depression. The subjects were 20 elderly stroke
patients assigned to experimental (n=10) or control (n=10) groups using convenience
sampling. The primary dependent variables were Maximum Phonation Time (MPT),
fundamental frequency (F0), voice intensity, fundamental frequency (jitter), frequency
perturbation (shimmer), and Noise-Harmonic Ratio (NHR). These measures, completed by
both experimental and control groups before and after the interventions, were recorded
and analyzed by the Praat program. Geriatric Depression Scale-Korean (GDS-K) was also
used to evaluate both groups on depression levels. Experimental subjects completed ten
30-minute individual sessions over a five-week period (twice a week) in which several
therapeutic vocal training interventions were utilized: respiratory training, vocalization
training, and therapeutic singing. As a result of this study, the experimental group
displayed a meaningful increase in MPT but a significant decrease in shimmer and NHR
in both pre- and post- tests after the interventions. Though statistically insufficient, the
increased measurement of intensity and the deceased measurement of jitter and F0 were
also found in the experimental group. Analyses of GDS-K showed no significant
differences between the groups while the experimental group displayed a slight decrease
of 0.72 in average. In conclusion, this study demonstrated that the therapeutic vocal
training promoted and suggested promising effects on the vocal quality of elderly stroke
patients. Therefore, it is highly recommended to implement the therapeutic vocal training
as a treatment program for elderly stroke patients.
