IntroductionSchwannomas are typically soft tissue tumors along sensory nerves which lie in the head, neck, mediastinum, and retroperitoneum or along the cranial nerves. Intraosseous schwannomas are account for less than 0.2% of primary bone tumours. They arise from the intraosseous nerve sheath cells and most commonly occur in the mandible.
we report a rare case of an intraosseous ancient schwannoma involving the L2 vertebra with its imaging findings and surgical aspects.
Case report
A 65-year-old male who had progressive lower back pain for 5 years was admitted to our department. At first He had controlled the pain by local po medication. However his pain increased gradually, the pain was almost more than he could bear. He presented Rt. hip, thigh pain and dorsum tingling sensation(Rt. L5 dermatome) with no sole numbness. He was no history of trauma and he was otherwise completely healthy.
The results of urinalysis and blood count, serum biochemical and electrolytic analyses were normal. He had no motor neurologic deficits and DTR(deep tendon reflex) revealed no abnomalities but he showed paresthesia on right L5 sensory dermatome.
The lumbar spine x-ray showed a about 4.5 cm osteolytic expansile mass with marginal sclerosis located at the right side of the L2 vertebra.(Fig.1-4)
On CT preoperative findings resembled that of any primary tumor of the bone aneurysmal cyst, osteosarcoma and bone metastasis. On computed tomography showed a about 4 x 3.7 x 3.3 cm osteolytic expansile mass with sclerotic margin involving right posterior lamina, right pedicle, right transverse process and superior articular process of L2.(Fig.5-7, Fig 8,9)
