Ⅰ. 서론세계 각국은 고유의 유일한 보건의료체계를 구축하고 있다고 해도 과언이 아니다. 각 나라마다 그 사회를 지배하는 이데올로기와 가치관에 따라 나름의 정치, 경제, 사회, 문화 및 행정적 구조 등 사회체계를 형성하고 있으며, 보건의료체계 역시 그 사회체계의 일부이기 때문이다.
그럼에도 불구하고 산업화된 선진국가들의 의료체계는 국민건강과 복지, 그리고 효율적인 의료자원의 관리를 위하여 방식과 정도는 다르지만 국가가 주체가 되어 의료체계를 관리한다는 점에서는 공통점을 갖고 있다. 즉 대부분의 선진국은 정부에 의해 운영되는 전국민건강보험(National Health Insurance, NHI) 또는 조세를 재원으로 하는 전국민보건서비스(National Health Service, NHS)를 통하여 대부분의 국민에게 보건의료서비스를 제공하고 있다.
그러나 미국에서는 모든 국민이 자동적으로 의료보험에 가입되지 않으며, 또한 정부가 대부분의 시민에게 의료서비스를 제공하지도 않는다. 미국은 전통적으로 개인의 자유를 보장하고 책임을 중시하여 정부의 통제를 최소한으로 하는 것을 원칙으로 하여 왔다. 보건의료제도 역시 의료서비스의 공급 및 의료재원의 조달 등에서 자유시장경제의 기본 특성을 가지며, 따라서 미국의 의료체계는 시장지향 체계의 대표적인 예인 셈이다. 이에 따라 의료체계를 구성하는 여러 가지 하부 요소들을 갖추고 있기는 하지만, 의료체계는 너무 분절화되어 있어 진정한 의미의 체계(system)라고 부르기 어려우며, 이는 미국 의료체계의 중요한 특질이 되고있는 것이다(Wolinsky, 988; Shortell et al., 996). 이러한 점에 주목하여 미국의 의료체계는 의료문제를 개인의 책임과 시장에 기반하여 해결하고 있는 유형인 자유기업형(Free Enterprise Pattern)으로 분류되고 있으며(Roemer, 993), 한편으로 소비자의 선택권을 최대한 보장한다는 의미에서 소비자주권형(Consumer Sovereignty Model)으로 분류되고 있다(OECD).
