수술실에서의 의료인들은 환자들이 수술 후에 폐 합병증을 예방하기 위해 수술실에서 병동에 올라가기까지 기침과 심호흡을 권장하고 있다. 그러나 마취에서 깨지 못한 환자들은 쉽게 볼수 있지만 특히, 노인 환자들에게서 젊은 환자들보다 제대로 된 기침과 심호흡을 이루어지지 않는 모습이 쉽게 관찰되었다. 수술 후 이러한 호흡운동이 과연 얼마나 폐환기능 및 폐 합병증예방이 탁월한 효과를 보이는지, 어떠한 호흡운동방법이ㅊ 더욱 효과적인지에 대해 연구하고자한다.

1) 연구의 필요성 및 목적
전신마취 수술 환자는 호흡기능 부전으로 인해 무기폐, 폐렴, 기관지염, 기관지경련, 폐부종, 상기도 폐쇄 등의 다양한 수술 후 폐 합병증을 경험하게 되는데, 특히 수술 후 폐 합병증의 주요 증가요인으로 60세 이상, 상복부 수술, 전신마취 등이다. 나이가 들수록 폐의 용적 중 폐쇄용적이 증가하고 근육강도와 폐유순도가 감소함으로 1초간 노력성 호기량이 감소하는데,1초간 노력성 호기량이 1.0L 미만은 수술 후 폐 합병증의 독립적인 위험 예측인자라 한다.
상복부 수술은 하복부 수술에 비해 폐 합병증이 1.5배 이상 높게 발생하는데, 수술 후에는 흡기 용량, 폐활량, 기능적 잔기용량이 감소하는 제한성 환기장애가 잘 나타나며, 통증이나 외과적 절개로 인해 흉벽과복벽의 기능도 저하될 수 있다.폐 합병증을 줄이기 위한 여러 가지 호흡훈련 방법 중에서 유발성 폐활량계를 이용한 흡기 호흡운동이 효과가 있음이 보고되어 임상에서 많이 활용되고 있지만, 근래 이루어진 연구 결과들은 근거가 명확하지 않다고 보고하고 있다.
수술 후 폐 합병증을 예방하고 환자의 폐환기능을 유지 · 증진시키기는 일은 중요한 간호중재이며, 특히, 노인 환자인 경우 쳬력저하 및 의사소통 어려움이 더 많고 폐 합병증의 위험이 커 보다 세심한 호흡관리가 요구된다.
