만삭아에서는 분당 90회 이하, 조산아에서는 분당 110회 이하로 정의할 수도 있다. 동결절에서 유래되는 부정맥인 동성 서맥은 만삭아에서 짧게 일시적으로 나타나는 경우가 많아 비인두 흡입, 먹을 때, 변을 볼 때, 딸꾹질할 때 나타난다. 대개 양성이고 신생아기 심장에는 부교감 신경이 더 발달되어 있는 것으로 이해될 수 있다. 길게 나타나는 것은 심한 전신 질환 즉 저산소증, 산혈증, 뇌압 증가 등의 경우에 나타난다.
대개 서맥이 나타날 경우 심장 질환을 일차적으로 생각하지 않는다. 그러나 심장질환을 고려할 때 가장 흔한 원인은 심방의 광범위한 수술 즉 폰탄(Fontan) 수술, 무스타드(Mustard) 혹은 세닝(Senning) 수술이다. 동결절을 직접 손상하거나 혈관 손상으로 인하여 수술 직후 나타나거나 수년 후에 나타난다.

동성 서맥을 의심되거나 확진한 경우 적어도 한번은 24시간 심전도를 할 필요가 있다. 심방성 빈맥이나 심실조기수축 등 다른 부정맥이 자주 동반되기 때문이다. 휴식 중 동성 서맥이 있는 경우에 운동검사는 동기능 장애 진단에 중요하다. 운동에 의한 심박수 증가의 저하 또는 운동 중에 증가된 박동수가 운동 중지 후 급격히 떨어지면 동기능 장애를 의미한다
