보디스캔이 뇌졸중 환자의 어깨관절 기능에 미치는 영향

본 연구는 뇌졸중 환자를 대상으로 보디스캔을 재활치료과정에 부가적으로 활용할 경우 손상측 어깨관절의 운동기능에 미치는 영향을 확인하는 것이다. 연구를 위해 재활병원에 입원한
뇌졸중 환자 28명을 선별하여 무작위 배분 후, 실험 집단에는 MBSR프로그램의 보디스캔을
본 연구의 목적에 맞게 재구성하여 적용하였으며, 비교 집단에는 휴식취하기를 실시하였다.
두 집단 모두 주5회, 매회 10분, 총 4주간 전통적 재활치료 실시 전 개별적으로 실시하였다.
보디스캔의 효과검증을 위해 디지털 듀얼경사계를 이용하여 손상 측 어깨의 굽힘과 벌림에
대한 어깨 관절가동범위 데이터를 수집하고 중재 전, 후 집단 간 차이를 비교하였다. 그 결과
어깨관절 굽힘과 벌림의 관절가동범위가 실험 집단, 비교 집단 모두에서 통계적으로 유의미
하게 증가한 것으로 나타났다(p<.001). 특히 어깨관절 벌림에 대한 관절운동범위는 실험 집단
이 비교 집단에 비하여 통계적으로 유의미하게 증가한 것으로 나타났다(p<.01). 이러한 결과
는 보디스캔을 전통적 재활치료에 부가하여 실시할 경우, 뇌졸중 환자의 손상 측 어깨관절에
영향을 미치며 나아가 어깨관절 기능회복에 효과가 있음을 시사한다.
영어 초록
The purpose of this study is to investigate the influence of Body Scan on functional recovery of shoulderjoint in stroke patients. Twenty-eight inpatients who were diagnosed as stroke participated with agreement
in this study, also they were divided randomly by experimental and control groups. Before traditional
therapeutic exercise, Body Scan was performed in experimental group, as well as control group took a
rest, respectively. Body Scan program was modified using Body Scan part of Mindfulness Based on Stress
Reduction (MBSR) for present study, then it was applied individually for 4 weeks, 5 days a week for 10
min. To determine the effects of this training program, data of shoulder joint movement were collected
and analyzed from Dualer IQ Inclinometer for flexion and abduction in affected shoulder joint. By
comparison of changes in range of motion, both experimental and control groups showed significant
increase of ROM from all two of Joint activity after therapeutic intervention (p<.001). In particular,
increment of abduction activity in experimental group was markedly higher than control group (p<.01).
These results may indicate that traditional therapeutic exercise combined with Body Scan result in increase
of ROM of shoulder joint, suggesting it is beneficial for functional recovery in stroke patients.
